I admire your sharing here with us and would like to offer some help!
You are right to first think of heredity for this can be in the genes, yet not an impossibility to deal with if you are willing to do the research and consult a doctor. Locksofluv has a good point on that. And yes, stress can be a factor as well as improper diet. However, I feel that it is probably a combination of things. Perhaps these, perhaps something more.
You need not answer here in the forum please, however, ask yourself what prescriptions are you taking? And what about any social habits like alcohol that can contribute. I say contribute. Would never want anyone to think I am accusing or judging. : )
Just some things for you to self ****yze.
When my father was very ill, he reeked of a bad odor due to the medications he was on. They were literally sweating out of the pores of his skin. This also means through the pores of his scalp known as follicles. Let's face it, medications, both over the counter and prescriptions that are taken over an extended period of time, can be toxic in many ways even those we need for certain illnesses. So anything introduced into our system that is not healthy or ideal for us could definitely play a part in weak, fragile hair or follicles.
Also, a large number of folks, mostly men, wear hats or ballcaps not allowing proper oxygen or sunshine to help aid in their health, therefore lack of vitamins such as "D".
Scalp stimulation is EEEsential! This could singly be the main problem for a lot of hair loss. Brushing or massaging the scalp in any way will get the blood flowing and allow said blood to do its job.
