I poor about a quarter size in my hand and start at the bottom, I don't know why I do this, just habit I guess. I have alot of thick hair and need to use alot just to get it too lather up.

I don't know if this was true or not. Im sure it was because a stylist told me, but I heard that the shampoo does not have to lather to mean it is cleaning your hair. Do you agree with this?

So massage your scalp with your fingertips and then wind your hair on top of your head? How long should you spend on the shampoo process?
This depends on the thickness of your hair. If you hair is thick, it will take at least two minutes. If it is thin and short, then 30-60 seconds should do it.

You actually wait unti you can know you have worked shampoo in all areas along the scalp and compilation of hair. From this point on, working lather in up to a minute should suffice.

I am going to disagree with this. She may have been thinking of a certain shampoo that may not lather up much. However, I will agree that some shampoos lather up more than others. I know that some people feel they have to have lots of lather and so they keep piling on the shampoo. This can be bad for if the shampoo is harsh, you're actually adding more harshness.

In general, if your hair is really dirty or oily, it isn't going to lather up much at all for that first shampoo. So then folks like to rinse and shampoo up again to make sure it's getting clean. Thing is, your hair was getting clean that first time around, you just didn't know it.

If you really like to have the lather, pick 'creamy' shampoos.

Then I guess I'm doing OK. I have small hands so a handful is the right amount of shampoo. Massaging for 30-60 seconds should be the right time to get clean. Now I just need a timer in the shower.

I have read that it is important to use cool to luke warm water to shampoo and condition your hair. Also put the shampoo into your hand before putting it in your hair for even distribution. My question, is there a rule to how long to rub the shampoo into your hair?

The hotter the water temperature, the more your hair is going to open up. Since the more it opens up, the more likely it is going to 'catch' and get tangly, you are correct that hot water is something to avoid. This is only going to be a problem for those folks who like hot, hot showers.

Usually the moderately warm water people probably use for a shower is fine. Rinsing the final rinse with cool water is perfect to help hair close back up. However, in the shower, that is not always preferred.

As for how long to shampoo, please refer to a post I did, two posts prior.
